Housing Coordinator (Home Sweet Home)

Position Overview:

The Housing Coordinator will play a pivotal role in aiding individuals and families experiencing homelessness to identify and transition into the Home Sweet Home Housing initiative.

This position collaborates closely with the program's case manager to address the housing requirements of participants navigating the Fresno-Madera Continuum of Care and Coordinated Entry System.


Key Responsibilities:

The essential functions of this role include:

  1. Evaluating the unique housing needs of all referred clients and collaborating with them to formulate personalized housing strategies while addressing any obstacles.
  2. Monitoring and assessing each client's progress through their housing search plan, making necessary adjustments as required.
  3. Creating and maintaining a comprehensive list of available housing options for clients.
  4. Assisting clients with housing applications, completing necessary supportive and subsidized housing documentation, surveying the rental market for affordable options, and advocating on behalf of clients with potential landlords.
  5. Acting as a liaison between program participants and property management entities or owners.
  6. Conducting bi-weekly unit inspections and processing maintenance requests as needed.
  7. Ensuring clients adhere to occupancy agreements.
  8. Managing monthly rent payments, if applicable.
  9. Maintaining client-related data tracking systems, including detailed case notes.
  10. Preparing reports on case-related outcomes, successes, challenges, and other necessary documentation.
  11. Engaging in outreach to realtors, landlords, housing developers, and other housing providers to identify new and existing housing opportunities.
  12. Networking with other agencies, coalitions, and participating in local community meetings.
  13. Actively participating in staff meetings and training sessions.
  14. Collaborating with the Community Coordinator/Matcher to effectively navigate housing solutions for individuals and families.
  15. Embodying the mission, vision, guiding principles, and goals of WestCare Foundation.
  16. Performing additional duties as assigned.

Qualifications:

To excel in this role, candidates must possess the following:

  • Exceptional communication and public speaking abilities.
  • Cultural and linguistic sensitivity towards the populations served.
  • Knowledge of substance abuse and treatment is advantageous.
  • Strong judgment and discretion.
  • Ability to thrive in a collaborative team environment.
  • Commitment to the highest ethical standards, particularly regarding confidentiality.
  • Professional demeanor and appearance.
  • Valid driver's license and the ability to maintain vehicle insurance as required.
  • Ability to pass background clearance checks.

Education and Experience:

Preferred qualifications include:

  • Two years of experience in public service.
  • One year of experience in property management.
  • A degree in Psychology, Social Work, Rehabilitation Counseling, or a related field is preferred; a high school diploma or equivalent is required.

Working Conditions:

This role is primarily conducted in a professional office environment within a treatment facility.

  • Effective communication, including verbal exchanges and active listening.
