Role SummaryThe Housing Coordinator and Support Manager will collaborate with local organizations, governmental bodies, and landlords to facilitate housing solutions, negotiate rental agreements, and establish move-in conditions for clients. This role includes overseeing lease contracts for various housing types, including short-term, transitional, and permanent arrangements.
Key ResponsibilitiesAccommodations may be provided for individuals with disabilities to fulfill essential job functions.
- Conduct regular housing inspections, adhere to reporting protocols, and assist clients in resolving any identified issues.
- Offer case consultation, crisis management, discharge planning, and targeted case management services to help clients meet their fundamental and housing needs.
- Ensure timely and accurate documentation as required by contracts and maintain minimum standards for billable client services.
- Identify housing options that align with clients' short-term, transitional, and permanent needs while managing lease agreements.
- Prepare and submit subsidy applications, maintain comprehensive client housing records, and verify the accuracy of rent calculations.
- Assist with monthly rent collection, transport clients to housing sites, and support them in obtaining necessary documentation for housing placement.
- Conduct housing evaluations and assessments in accordance with the coordinated entry system.
- Participate in on-call coverage as needed.
- Deliver field-based services based on program requirements and client needs.
- Maintain confidentiality regarding client care and participate in scheduled program meetings.
- Perform other relevant duties as assigned.
- Bachelor's degree with 1 year of experience, an associate degree with 3 years of experience, or 5 years of relevant experience.
- High School Diploma or GED is required.
- A valid driver's license and current auto insurance are mandatory.
- Preferred knowledge of housing needs for individuals with behavioral health challenges.
- Cultural competency is essential.
- Bilingual candidates are preferred.
- Familiarity with Microsoft Office and EHR software is advantageous.
