The Housing Case Manager will play a vital role in assisting clients with the development of social and community support systems to promote stability and enable clients to function at the highest level possible.

Key Responsibilities

  • Coordinate services for clients to ensure seamless support and guidance.
  • Provide independent life skills training, group education, and family activity coordination to empower clients.
  • Assist clients with gathering essential documents for housing and navigating the housing application process.
  • Locate and secure suitable housing opportunities in the community, taking into account client needs and preferences.
  • Maintain accurate and up-to-date client files, adhering to documentation deadlines and requirements.
  • Complete training and maintain proficiency in data entry and community assessments, such as the VI-SPDAT.

Requirements

  • Bachelor's Degree in social services or a closely related field, preferred.
  • At least one year of experience in housing-related social services.
  • A clean driving record.
