Role SummaryThe Housing Specialist/Case Manager will collaborate with local organizations, governmental bodies, and landlords to facilitate housing solutions for clients. This role involves negotiating rental agreements, deposits, and terms of occupancy on behalf of clients while overseeing lease arrangements for various housing types.
Key Responsibilities- Conduct regular housing inspections, adhere to reporting protocols, and assist clients in resolving any identified issues.
- Provide comprehensive case management, including crisis intervention and discharge planning, to help clients achieve their housing and basic needs.
- Ensure timely and accurate documentation as required by contracts and maintain minimum standards for billable client services.
- Identify housing options that cater to short-term, transitional, and permanent needs while managing lease agreements effectively.
- Prepare and submit subsidy applications, maintain organized client housing records, and verify the accuracy of rent calculations.
- Assist with monthly rent collection, transport clients to housing sites, and support them in obtaining necessary documentation for housing placement.
- Conduct housing assessments and screenings in accordance with the coordinated entry system.
- Participate in on-call coverage as needed.
- Deliver field-based services as dictated by program requirements and client needs.
- Uphold the confidentiality of client information.
- Engage in scheduled program meetings and other relevant duties as assigned.
- Bachelor's degree with 1 year of relevant experience, or an associate degree with 3 years of experience, or 5 years of applicable experience.
- High School Diploma or GED is mandatory.
- A valid driver's license and current auto insurance are required.
- Familiarity with the housing needs of individuals facing behavioral health challenges is preferred.
- Understanding of cultural competency is essential.
- Bilingual candidates are preferred.
- Proficiency in Microsoft Office and EHR software is advantageous.
