Position Overview:
The individual in this role will be tasked with aiding individuals and families experiencing homelessness in locating and transitioning into a diverse array of permanent housing solutions, including subsidized options such as Housing Choice Voucher, Shelter Plus Care, VASH, and other supportive housing programs.

Key Responsibilities:
  1. Evaluate the unique housing requirements of all referred clients and collaborate with them to formulate personalized housing strategies while addressing any obstacles.
  2. Track and assess each client's progress through their housing search plan, making necessary adjustments to the plan as required.
  3. Compile and maintain a comprehensive list of available housing alternatives for clients.
  4. Assist clients in completing housing applications, manage supportive and subsidized housing documentation, research the rental market for affordable options, and advocate on behalf of clients with potential landlords.
  5. Stay informed about the regulations governing federal housing programs involved in the supportive housing system.
  6. Maintain accurate client-related data tracking systems, including case notes and necessary entries.
  7. Generate reports related to cases, including outcomes, successes, challenges, and other required documentation.
  8. Ensure complete and precise documentation of all service objectives and their outcomes in accordance with relevant guidelines.
  9. Engage with realtors, landlords, housing developers, and other housing providers to discover new and existing housing opportunities.
  10. Network with other organizations, coalitions, and participate in local community meetings.
  11. Actively engage in staff meetings and training sessions.
  12. Collaborate effectively with the Community Coordinator/Matcher to assist individuals and families in navigating housing options.
  13. Perform additional duties as assigned.

Qualifications:
  • Exceptional communication and public speaking abilities.
  • Preferred experience of one (1) year in a supervisory role.
  • Minimum of two (2) years in case management services.
  • Proven capability to work with underserved populations.
  • Experience in conducting outreach, including street outreach.
  • Proficient computer skills.
  • Strong documentation skills.
  • A valid driver's license and the ability to obtain and maintain vehicle insurance.
  • Ability to manage multiple tasks and make independent decisions.
  • CPR Certification, First Aid Certification, and an Annual Tuberculosis test are required.

Education:
A High School diploma or equivalent is mandatory.

Work Environment:
The role involves a combination of community engagement and office work, with some travel required.

Physical Requirements:
Occasional lifting of over 10 pounds; routine activities include sitting, standing, walking, and reaching. The position typically does not involve exposure to blood, body fluids, or tissues, although there may be potential exposure as part of the job responsibilities. Personal protective equipment will be provided for employees engaged in relevant tasks.
