As a key member of our care coordination team, you will be responsible for providing direct services and day-to-day direction to a team of care managers. Your expertise will be instrumental in coordinating client care among multiple providers, both internal and external, to ensure seamless transitions and optimal outcomes.

Key Responsibilities
  • Care Coordination: Develop and implement comprehensive care plans that meet the requirements of health plans and address identified barriers.
  • Team Leadership: Provide day-to-day direction and supervision to a team of care managers, ensuring that they have the necessary resources and support to excel in their roles.
  • Client Advocacy: Advocate for clients' voices and choices, ensuring that their needs are met and their rights are respected.
  • Documentation and Reporting: Maintain accurate and timely documentation of client files and complete required reports.
  • Collaboration and Communication: Foster strong relationships with clients, families, and healthcare providers to ensure effective communication and coordination of care.
Requirements
  • Education: Bachelor's degree in social services or a closely related field.
  • Experience: 3 years of experience in housing-related social services, with a focus on care coordination and case management.
  • Skills: Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations and stakeholders.
  • Certifications: Ability to bill Medi-Cal and maintain a clean driving record.
