Position Overview

SUMMARY

Collaborate with local educational institutions, community organizations, and internal staff to assess and address immediate and ongoing housing needs for young adults. Engage in outreach efforts, providing personalized support to homeless and unaccompanied youth in the surrounding areas.

KEY RESPONSIBILITIES

  • Assess all new referrals and walk-in youth requiring assistance.
  • Conduct comprehensive intake assessments and collaborate with clients and case management teams to create and implement rehousing strategies.
  • Evaluate housing and support service requirements to identify applicable short-term financial assistance programs for rapid rehousing.
  • Guide clients in gathering necessary documentation and completing housing-related applications.
  • Assist clients in filling out housing applications and negotiating lease agreements with property owners.
  • Build and maintain relationships with landlords, focusing on establishing connections with low-barrier housing providers, while keeping an updated list of available properties.
  • Compile a comprehensive directory of housing resources across various regions to support families with existing ties.
  • Work in partnership with various service providers, including governmental and community organizations, to ensure seamless service delivery.
  • Actively participate as a member of the team; attend all required meetings and training sessions.
  • Maintain detailed client records, including completed intakes, rehousing plans, referrals, and services utilized.
  • Ensure compliance with all relevant federal, state, and local funding regulations, as well as agency policies.
  • Be prepared to travel within the region to meet with service providers, clients, and landlords, as well as to attend meetings.
  • Collect necessary data, maintain accurate client documentation, and provide regular updates on client progress, demographics, challenges, and new resources.
  • Perform other related duties as assigned.

QUALIFICATIONS AND SKILLS

  • Experience in providing vocational and educational support.
  • Strong assessment, advocacy, and case management abilities.
  • Ability to maintain a flexible work schedule.
  • Bilingual candidates are preferred.
  • Reliable transportation is required.
  • A valid driver's license is necessary.

PHYSICAL DEMANDS

  • Must be capable of conducting outreach activities, which may involve extended periods of walking and standing while engaging with clients.

WORK ENVIRONMENT

Office space is available within the staff office and community locations such as shelters and educational institutions. Frequent travel throughout the area is expected. This role involves direct interaction with the young adult demographic. Business casual attire is required. A successful background check is mandatory.
