POSITION OVERVIEW


Collaborate with local educational institutions, community organizations, and South Middlesex Opportunity Council (SMOC) personnel to identify urgent and immediate requirements that promote housing stability.

Engage in outreach efforts, perform assessments, and provide individualized support to the homeless young adult and unaccompanied youth demographic in the MetroWest region.

KEY RESPONSIBILITIES
Evaluate all new referrals and walk-in youth requiring assistance.

Conduct intake sessions with each client and collaborate with the client and case management team to formulate and implement a rehousing strategy.


Assess housing and support service requirements to identify which short-term financial assistance programs may be accessible for prompt rehousing.

Assist clients in gathering necessary documentation and completing applications related to housing.

Support clients in filling out housing applications and negotiating lease agreements with property owners.


Establish connections with landlords, focusing on building relationships with new, low-barrier landlords, and maintain an updated directory of landlords and properties.


Create a comprehensive list of housing resources across various regions of the state where families may have connections and support systems.


Work in partnership with various service providers including the Department of Transitional Assistance (DTA), the Department of Housing and Community Development (DHCD), the Department of Children and Families (DCF), the Department of Youth Services (DYS), Early Intervention, Legal Services, and Behavioral Health Services (BHS) to ensure seamless service coordination.

Participate as an integral member of the CGRC team; attend all staff meetings and other required gatherings.

Maintain client records to include completed intakes, rehousing plans, referrals made, and services utilized.


Ensure that services comply with all federal, state, and local funding regulations, as well as agency policies and procedures.


Be prepared to travel throughout the region to meet with service providers, potential clients, and landlords, and to attend meetings.


Collect necessary data, maintain accurate client records, and provide regular written updates including client progress, demographics, barriers, and new resources.

QUALIFICATIONS AND SKILLS
Experience in providing vocational and educational support.

Strong assessment, advocacy, and case management abilities.

Flexible schedule is essential.

Bilingual candidates are preferred.

Reliable transportation is required.

A valid Massachusetts Driver's license is mandatory.

PHYSICAL DEMANDS


Must be capable of conducting street outreach, which may involve extended periods of walking and standing while engaging with clients.


WORK ENVIRONMENT


Workspace is provided within the CGRC staff office and community satellite locations such as shelters and educational institutions.

Frequent travel throughout the MetroWest area is necessary.

As part of this role, the Housing Resource Navigator will have direct interaction with the young adult population.

Business casual attire is expected. A successful background check is a prerequisite.
