Job Title: Medical Social Worker - Patient Care Specialist

White Plains Hospital Center is seeking a highly skilled Medical Social Worker - Patient Care Specialist to join our team.

Job Summary:

The Medical Social Worker - Patient Care Specialist will provide comprehensive social work services to patients and their families, including assessment, counseling, and advocacy. The ideal candidate will have a strong understanding of the healthcare system and be able to navigate complex medical and social issues.

Key Responsibilities:
  • Assessment and Intervention: Conduct thorough assessments of patients' social and emotional needs, and develop and implement individualized plans to address these needs.
  • Counseling and Support: Provide counseling and support to patients and their families, addressing issues such as grief, anxiety, and depression.
  • Advocacy and Referral: Advocate for patients' needs and interests, and provide referrals to community resources and services.
  • Collaboration and Communication: Work closely with healthcare teams to ensure comprehensive care, and communicate effectively with patients, families, and healthcare providers.
  • Documentation and Reporting: Maintain accurate and timely documentation of patient interactions and progress, and provide regular reports to healthcare teams and management.
Requirements:
  • Education: Master of Social Work (MSW) degree from a Council on Social Work Education (CSWE) accredited program.
  • Experience: At least 2 years of experience in a healthcare setting, preferably in a social work or patient advocacy role.
  • Licensure: Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) in the state of New York.
  • Certification: NYS SCREEN Certification.
