  • Provide supportive counseling, crisis intervention, and grief counseling to patients and their families as needed.
  • Develop and implement social work treatment plans, documenting progress and outcomes in patient records.
  • Provide information, referral, and advocacy to community agencies, ensuring patients receive necessary resources and support.
  • Advocate for patients' rights and interests, serving as a liaison between patients, families, and healthcare providers.
  • Participate in individual and team case conferences, sharing expertise and contributing to patient care planning.
  • Supervise and mentor social work assistants and students, providing guidance and support in their professional development.
  • Lead education and support groups for patients and families, promoting health education and community engagement.
  • Identify and respond to cases of abuse, neglect, or exploitation, ensuring patients receive necessary support and resources.
Requirements
  • Master of Social Work (MSW) degree from a Council on Social Work Education (CSWE)-accredited program.
  • Two years of experience in an acute care hospital setting or related field.
  • Licensed Social Worker (LMSW/LCSW) certification approved by the New York State Education Department.
  • NYS SCREEN Certification (obtained within initial 3 months of employment).
