Job Summary:

A Hospice Clinical Liaison at Kindful Health LLC is a vital member of our team. The Clinical Liaison is responsible for establishing and developing new business relationships, maintaining current connections, and creating and implementing a sales and marketing strategy to cover the assigned sales/marketing region.

Key Responsibilities:
  • Generate Qualified Referrals: Develop and execute a plan to achieve hospice admissions and meet short and long-term target account goals.
  • Maintain Referral Records: Keep up-to-date records of referral sources, including physicians, hospitals, skilled nursing facilities, senior living facilities, discharge planners, and case managers within an assigned territory.
  • Assist with Referral Process: Help the care center obtain necessary documentation to complete the referral to admission process.
  • Evaluate Patient Referrals: Assess patient referrals to determine suitability for hospice admission.
  • Provide Consultation and Counseling: Offer consultation and counseling to referrers, patients, and families about Kindful's hospice care options.
  • Coordinate Patient Care: Communicate patient health, financial, psycho-social, spiritual, medical, and DME needs to the care team and coordinate with the care team to ensure Kindful allocates appropriate resources to meet patient needs.
  • Liaison Services: Act as a liaison to patients and families during the transitional period between patient discharge and the initiation of hospice services.
  • Sales and Marketing: Master the essentials of the sales model and utilize the sales process daily.
  • Client Relationships: Develop and maintain client relationships within a competitive marketplace.
  • Business Planning: Establish and promote ongoing collaborative relationships with referral sources, institutional personnel, and other facilities to ensure the coordination of appropriate referrals.
  • Account Prioritization: Prioritize accounts in accordance with the market sales plan.
  • Customer Service: Respond to customer complaints in a timely manner.
