Job Summary: The Clinical Sales Liaison at Kindful Health plays a crucial role in our organization. This position is tasked with establishing and nurturing new business relationships, sustaining existing connections, and crafting a comprehensive sales and marketing strategy for the designated region. The Clinical Liaison should be adept at engaging with patients, families, healthcare professionals, and external partners.

Essential Education and Experience: An Associate's degree is required, along with a minimum of two years of experience in hospice sales or clinical settings. The Clinical Liaison must possess and maintain a valid LVN or RN license.

Responsibilities & Duties:

  • Generate qualified referrals to achieve hospice admissions in alignment with both short-term and long-term objectives.
  • Maintain an accurate and comprehensive record of referral sources, including healthcare providers, hospitals, and senior living facilities, within the assigned territory using CRM tools.
  • Assist the care center in gathering all necessary documentation to facilitate the referral-to-admission process.
  • Assess patient referrals to determine their suitability for hospice care.
  • Provide guidance and support to referrers, patients, and families regarding Kindful's hospice services.
  • Communicate all patient needs—health, financial, psycho-social, spiritual, medical, and DME—to the care center and collaborate with the care team to ensure appropriate resources are allocated.
  • Act as a liaison for patients and families during the transition from discharge to the initiation of hospice services.
  • Master the sales model and apply the sales process consistently.
  • Develop and sustain client relationships in a competitive landscape.
  • Establish and foster ongoing collaborative relationships with referral sources and institutional personnel to ensure effective coordination of referrals.
  • Prepare business plans and maintain target lists, prioritizing accounts based on the market sales strategy.
  • Address customer concerns promptly and effectively.
  • Perform additional duties as assigned.
