POSITION OVERVIEW

The Community Outreach Coordinator plays a crucial role in fostering and nurturing relationships with clients, leading to increased referrals and admissions. This position is dedicated to enhancing the organization's reputation within the community by engaging with healthcare professionals, including physicians, hospitals, nursing homes, and other care facilities, as well as discharge planners and case managers to advocate for our hospice services.

KEY RESPONSIBILITIES
  • Build and maintain professional connections with healthcare providers, discharge planners, social workers, and decision-makers through continuous education and effective marketing strategies.
  • Inform the community, referral sources, patients, and their families about hospice services through organized activities such as in-services, individual presentations, and group discussions.
  • Collaborate with the Director of Business Development and/or Executive Director to formulate, implement, and assess strategic marketing initiatives aimed at increasing patient enrollment.
  • Secure referrals and ensure timely delivery of educational resources and consent documentation to families and caregivers.
  • Proactively develop and sustain relationships within the admissions team and clinical staff to guarantee effective communication for our referral partners.
  • Ensure adherence to corporate and regulatory standards, focusing on business growth, professional development, and ethical practices.
  • Plan and execute public education and outreach campaigns to enhance community engagement.
  • Represent the organization at various community and business gatherings to advocate for our services.
  • Achieve or surpass established goals for admissions, patient census, revenue, and expenses within the designated area.
  • Consistently embody and promote the core values of the organization.

QUALIFICATIONS (Education, Experience, Skills & Abilities)
  • A Bachelor's Degree from an accredited institution is required.
  • A minimum of two years of experience in referral development, marketing, or sales within a healthcare setting is preferred.
  • Established relationships with local healthcare facilities or professionals, demonstrating a successful track record.
  • Strong communication skills to effectively engage with individuals from diverse socioeconomic backgrounds.
  • Comprehensive understanding of hospice philosophy and eligibility criteria.
  • Familiarity with the roles of various disciplines involved in providing hospice care.
  • Knowledge of HIPAA regulations and the importance of maintaining patient confidentiality.
  • Exceptional verbal and written communication abilities.
  • Strong organizational skills and the capacity to work independently while adapting to changing priorities.
  • Understanding of the role and limits of a patient advocate.
  • Self-driven with a strong desire to achieve success.
  • Possession or ability to quickly obtain a Level One Fingerprint Clearance card.
  • Reliable transportation for work purposes, including valid registration, current auto insurance, and a valid driver's license.
  • Proficiency in Microsoft Office applications, including Word, Excel, and Outlook.
  • Awareness of community resources available to support patients and families.
