Position Overview:


The Enrollment Support LVN plays a pivotal role in enhancing communication regarding the clinical care requirements for potential PACE participants. This position contributes to the organization's growth by leveraging clinical expertise to ensure eligibility validation and the attainment of enrollment objectives.


Through home assessments during the pre-enrollment phase, the Enrollment Support LVN engages with prospective participants and their families to evaluate home safety, gather information on Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs), assess the functional and social environment, and review medical histories and medications. While this role does not involve direct clinical care, it necessitates a solid understanding of the care needs of frail seniors.


Responsibilities include adhering to the Nurse Practice Act, CMS PACE Regulations, and the Center for Elders' Independence Code of Conduct, alongside maintaining professional educational standards.


The LVN may also assist the Community Access and Development team and collaborate with the Pre-Enrollment RN on outreach initiatives, which may involve identifying referral partners within the healthcare community, visiting key stakeholders, and providing educational sessions or participating in health fairs.


Compensation:
The salary range for the Enrollment Support LVN position at Center for Elders' Independence is

$30.10 to $45.15 per hour.

This hourly rate is influenced by market conditions as well as the candidate's experience, skills, and work history.

Key Responsibilities:

  • Conduct home visits as assigned, utilizing nursing expertise to perform safety evaluations, medication reconciliation, and validation of ADLs and IADLs while ensuring compliance with documentation standards.
  • Assist team members in achieving monthly enrollment goals.
  • Foster relationships with key community accounts through assigned visits.
  • Educate community members, potential participants, and caregivers about the PACE program.
  • Promote excellent customer service through timely communication, active listening, and a positive demeanor.
  • Collaborate effectively with the local center team, including managers, directors, support staff, and clinical personnel.
  • Support initiatives that enhance referral and enrollment development.
  • Contribute to marketing efforts by providing insights for the creation of new tools and messaging for external audiences.
  • Identify and promote educational opportunities related to the PACE model of care and assist in delivering presentations to referral partners.
  • Respond promptly to communications from supervisors and team members.
  • Maintain confidentiality in handling patient information and medical records, adhering to all HIPAA regulations.
  • Perform other related duties as assigned.


Qualifications:
Active LVN/LPN license in good standing in California.
Minimum of two years' experience in community-based healthcare or hospital settings required.
Preferred: at least one year of experience working with the elderly population.

Bilingual:
Preferred: Spanish/English or English/Cantonese/Mandarin.

Demonstrated clinical competency and technical skills for elderly patient care.
Strong verbal and written communication skills, with proven interpersonal and presentation abilities.

Excellent organizational and analytical skills in participant care environments.
Knowledge of regulatory requirements for conducting in-home safety assessments, medication reconciliation, and proper documentation.


Additional Requirements:
Ability to perform physical tasks such as sitting, standing, lifting (up to 10 pounds), and using a computer.
Travel may be required for site visits and special functions.

Work Environment:
Work may be conducted in community settings, patient homes, or PACE Centers, with potential exposure to outdoor environments during travel. Office work is performed in a climate-controlled environment.
Must possess a valid driver's license, reliable transportation, and proof of auto insurance.
