- Provide education and support to patients and their families on disease management, medication adherence, and healthy lifestyle choices.
- Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing strategies for enhancement.
- Administer medications and treatments as prescribed by physicians, ensuring accurate dosing and documentation.
- Monitor patients' vital signs, track progress, and report any changes or concerns to the healthcare team.
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
- Graduate of an accredited school of vocational nursing.
- Current LVN licensure in California.
- Basic Life Support (BLS) certification.
- Ability to work in a fast-paced environment, prioritizing tasks and managing multiple responsibilities.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Strong problem-solving and critical thinking skills, with the ability to analyze complex situations and develop effective solutions.
- Ability to maintain confidentiality and handle sensitive information with discretion.
- Minimum 1 year of professional experience in home health or community health.
- Direct patient care experience within the past five years.
- Basic competencies in the fundamentals of nursing and pharmacology.
- Ability to use and give instruction on adaptive equipment.
- A competitive hourly rate of $34.90 to $46.87.
