POSITION: VOCATIONAL NURSING SPECIALIST (Full or Part Time)

REPORTS TO: Director of Nursing, Family Bridges, Inc.

WORK SCHEDULE: Full-time (40 hours per week) or Part-time (30 hours per week)

EMPLOYMENT STATUS: Non-exempt (3 months orientation period)

APPLICATION STATUS: Open until filled

JOB OVERVIEW: Under the supervision of the Director of Nursing, the Vocational Nursing Specialist delivers direct nursing services to clients at the Community Based Adult Service Center.

KEY RESPONSIBILITIES:

  • Evaluates clients' requirements and formulates suitable nursing care in partnership with the RN during initial assessments and subsequent evaluations.
  • Administers direct nursing care, which includes medication management, chronic illness monitoring, health evaluations, acute care, assistance with daily living activities, and triage.
  • Maintains and updates Medication Administration Records (MAR), ensuring compliance and accuracy in self-medication practices according to regulatory standards.
  • Initiates preventive and rehabilitative nursing actions as necessary for the clients' care and safety.
  • Collaborates with the RN to create individualized care plans and assists in implementing these plans.
  • Provides precise and relevant documentation essential for recording both episodic and routine care.
  • Partners with the RN to deliver health education to clients, staff, and family members.
  • Coordinates with primary healthcare providers regarding patient care and follow-up.
  • Participates in intake and assessment meetings as appropriate, contributing to care planning as a member of the multidisciplinary team.
  • Performs additional duties as assigned by the Supervisor.

REQUIREMENTS:

  • Completion of an accredited vocational nursing program.
  • Possession of a current California LVN license.
  • A minimum of 6 months of experience in acute care, with familiarity in geriatric and long-term care issues.
  • Ability to function effectively within a multidisciplinary care environment.
  • Bilingual proficiency in English and Cantonese, Mandarin, or Vietnamese is preferred.

ADA COMPLIANCE:

  • Ability to see and hear clearly to interact with others.
  • Effective communication skills, both written and verbal.
  • Proficient in computer usage.
  • Capability to lift a minimum of 20 lbs.

SALARY RANGE: The target hourly rate for this position is $37.00, inclusive of benefits. The specific compensation will be determined based on the selected individual's education, experience, and skill set.
