POSITION: Licensed Vocational Nurse (LVN) - Available for Full or Part-Time
REPORTS TO: Director of Nursing, Hong Fook Community Based Adult Services
WORK SCHEDULE: Full-time (40 hours per week) or Part-time (30 hours per week)
EMPLOYMENT STATUS: Non-exempt (includes a 3-month orientation period)
JOB OVERVIEW: The LVN will deliver direct nursing services to participants at the Community Based Adult Service Center under the supervision of the Director of Nursing.

KEY RESPONSIBILITIES:
  • Evaluate participants' needs and collaborate with the RN to devise nursing care plans.
  • Administer direct nursing care, including medication management and health evaluations.
  • Keep precise medication records and ensure adherence to licensing standards.
  • Initiate preventive and rehabilitative nursing procedures for participant care.
  • Assist in the creation and execution of individualized care plans.
  • Document all provided care accurately and promptly.
  • Engage in health education initiatives for participants, staff, and family members.
  • Coordinate with primary physicians regarding patient care and follow-up.
  • Participate in assessments and care planning meetings with the multidisciplinary team.
  • Perform additional duties as assigned by the Supervisor.
REQUIREMENTS:
  • Graduate from an accredited LVN program.
  • Possession of a current California LVN license.
  • At least 6 months of experience in acute care settings.
  • Ability to thrive in a multidisciplinary care environment.
  • Bilingual in English and Cantonese, Mandarin, or Vietnamese is preferred.
ADA COMPLIANCE:
  • Effective communication skills, both written and verbal.
  • Proficient in computer operation.
  • Capability to lift a minimum of 20 lbs.
SALARY RANGE: The anticipated hourly wage for this position is $37.00, inclusive of benefits; final compensation will be determined based on individual qualifications.
