Job Overview

InGenesis is looking for a Licensed Vocational Nurse (LVN) specializing in Wellness Case Management to collaborate with our client. In this position, you will assess, strategize, and deliver case management services, along with health education and counseling for patients facing complex chronic conditions, including but not limited to Diabetes, Hypertension, Congestive Heart Failure, and Asthma, in alignment with BACH's Chronic Care model.

Key Responsibilities
  • Conduct a range of intricate, professional, analytical, and confidential case management services.
  • Maintain a registry for chronic diseases, entering, tracking, and updating patient data.
  • Schedule appointments and send reminders for routine and additional visits, as well as preventive care.
  • Review the registry for overdue laboratory tests.
  • Utilize the registry to identify patients for chronic disease management activities, ensuring they attend regular visits and complete overdue preventive care actions.
  • Establish and implement a system for tracking referrals, ensuring clients follow up on referrals according to established protocols.
  • Assist with eligibility assessments to guarantee access to necessary services.
  • Perform messenger duties, such as delivering brief messages regarding refills or appointment scheduling, and reminding patients to bring their medication bottles to visits.
  • Maintain ongoing communication with other clinical personnel involved in panel management.
  • To meet patient and client needs, you may be required to work at other BACH locations within a 50-mile radius.
  • Carry out additional duties as assigned.
Minimum Qualifications
  • High School Diploma.
  • Completion of an LVN program.
  • Current LVN license in California.
  • 1–2 years of relevant experience in a healthcare setting.
  • Active phlebotomy certification.
  • Reliable transportation access.
  • Proficiency in Microsoft Office and internet usage is essential.
Preferred Qualifications
  • Associate or Bachelor's degree in a related field.
  • Experience in a community health center, long-term care facility, or public health environment.
  • Experience working with patients with chronic diseases, particularly Diabetes and Hypertension.
  • Experience in outreach and presentations.
Physical Requirements

This role necessitates:

  • Activities such as sitting, walking, and standing typical of a standard medical/clinical office environment, including some bending, stretching, and lifting.
  • Manual dexterity for operating standard office equipment and computer keyboards.
  • Traveling to various work sites, which may involve exposure to outdoor elements.
  • Presenting information to groups.
  • Reading and writing skills for reviewing records for accuracy.

There may be exposure to blood or blood-borne pathogens, necessitating OSHA training.
