Licensed Vocational Nurse IV

The Licensed Vocational Nurse IV - Certified is a key member of the healthcare team, responsible for providing high-quality patient care in a Long Term Care or Sub-Acute setting. This role requires a strong commitment to delivering exceptional patient outcomes, while working collaboratively with other healthcare professionals to achieve this goal.
Key Responsibilities- Data Collection and Reporting: Perform ongoing and systematic data collection, focusing on physiologic, psychological, spiritual, and cognitive status, and report to the Registered Nurse.
- Plan of Care Development: Contribute to and update a plan of care, prioritized and based on determined nursing diagnosis and resident outcomes, and implement care in a knowledgeable, skillful, consistent, and continuous manner.
- Documentation and Communication: Document the effectiveness of the plan of care in progressing the resident toward positive outcomes, and collaborate with the Registered Nurse to identify resident/Primary Support Person (PSP) needs and participate in teaching resident/PSP education.
- Adherence to Safety and Legal Requirements: Demonstrate adherence to safety and legal requirements established at the hospital and department level, and demonstrate awareness and working knowledge of unit structure standards, nursing policies and protocols, and administrative policies.
- Professional Development: Recognize own limitations and education needs, improving knowledge and skill by participating in in-service and continuing education classes, and participate in unit goal formation and assist in projects to achieve goals.
- Effective Communication: Communicate effectively with co-workers, physicians, residents, and residents' PSP, formulating and maintaining effective working relationships, and demonstrate ability to organize workload and set priorities.
- Education: Graduate of an accredited vocational school of nursing.
- Experience: One year recent experience as a LVN in a Long Term Care or Sub-Acute setting.
- Licenses/Certifications: Current licensure in the State of California to practice vocational nursing, Basic Life Support (BLS) certification issued by American Heart Association, and IV Certification from Board of Vocational Nursing and Psychiatric Technicians.
