SUMMARY:

The AHD Registered Nurse at South Shore is responsible for delivering direct nursing care to residents while overseeing the care provided by certified nursing assistants (CNAs) and licensed vocational nurses (LVNs). This role demands adherence to all applicable federal, state, and local regulations, as well as facility guidelines.

The facility operates continuously, necessitating flexible work hours and days.

Additional duties may be assigned as necessary.


DUTIES & ESSENTIAL JOB FUNCTIONS:

NOTE: The following responsibilities are typical for this classification; however, not all duties may be performed by every individual in this role.

  • Employs a systematic approach to deliver personalized nursing care, ensuring it is knowledgeable, skillful, consistent, and continuous. Prioritizes patient care based on essential needs and available resources.
  • Coordinates the admission, discharge, and transfer processes for residents. Conducts resident rounds every two hours to evaluate physical and emotional well-being and to initiate necessary nursing interventions. Performs health assessments and accurately documents residents' symptoms and responses.
  • Formulates nursing diagnoses that guide the care strategy. Implements appropriate nursing interventions to stabilize residents' conditions and prevent complications, while evaluating their responses to these interventions.
  • Consults with physicians or licensed independent practitioners to clarify any orders or treatment plans that may appear inaccurate or ineffective. Understands the rationale behind medications and treatments, ensuring correct administration.
  • Reviews medication and treatment records for accuracy and compliance with physician orders. Administers and documents medications, enteral nutrition, IV therapy, and treatments as per physician directives, ensuring all care is accurately recorded.
  • Designs and facilitates resident education and counseling based on health needs, collaborating with residents, healthcare team members, and, when appropriate, residents' significant others.
  • Plans, organizes, and manages nursing rehabilitation/restorative care, initiating interventions that promote residents' independence. Documents specific nursing interventions in care plans and clinical records.
  • Supervises nursing staff involved in rehabilitation/restorative activities and educates nursing assistants on effective nursing practices.

Coordinates nursing interventions with therapy services (PT, OT, ST, RT) and ensures effective communication regarding residents' statuses, addressing family concerns and documenting resolutions.


MINIMUM QUALIFICATIONS:
Education:
Graduation from an accredited nursing program is required.
Preferred Education:
Bachelor's degree in Nursing.
Experience:
A minimum of one year of relevant experience is preferred, although new graduates will be considered.
Licenses/Certifications:
Must possess a valid Registered Nurse license in the State of California and current BLS – Basic Life Support certification.
