Key Responsibilities
  • Conduct a range of intricate, professional, analytical, and confidential case management services.
  • Manage a chronic disease registry, ensuring accurate entry, tracking, and updating of patient data.
  • Schedule appointments and send reminders for routine and ancillary visits, as well as preventive care.
  • Monitor the registry for overdue laboratory tests.
  • Utilize the registry to identify patients for chronic disease management activities, ensuring they attend regular visits and complete overdue preventive care actions.
  • Establish and implement a tracking system for referrals, ensuring clients follow up according to established protocols.
  • Assist in determining eligibility to guarantee access to necessary services.
  • Perform messenger duties, such as delivering brief messages regarding refills or appointment scheduling, and reminding patients to bring their medication bottles to visits.
  • Maintain consistent communication with other clinical staff involved in panel management.
  • To meet patient and client needs, you may be required to work at various BACH locations within a 50-mile radius.
  • Execute additional duties as assigned.
Minimum Qualifications
  • High School Diploma.
  • Completion of an LVN program.
  • Current LVN license in California.
  • 1–2 years of relevant experience in a medical setting.
  • Active phlebotomy certification.
  • Reliable transportation.
  • Proficiency in Microsoft Office and internet navigation is essential.
Preferred Qualifications
  • Associate or Bachelor's degree in a related field.
  • Experience in a community health center, long-term care facility, or public health environment.
  • Experience working with patients suffering from chronic diseases, particularly Diabetes and Hypertension.
  • Background in outreach and presentation.
Physical Requirements

This role involves:

  • Engaging in sitting, walking, and standing typical of a medical/clinical office environment, including some bending, stretching, and lifting.
  • Utilizing manual dexterity for standard office equipment and computer keyboards.
  • Traveling to various work sites, which may involve exposure to outdoor elements.
  • Presenting information to groups.
  • Reading and writing to ensure accuracy in record reviews.

May involve working with blood or blood-borne pathogens, necessitating OSHA training.
