We are seeking a highly skilled LVAD Program Coordinator to join our team at Wellstar Health System. The successful candidate will be responsible for coordinating the development of LVAD Program processes, guidelines, policies, and procedures. This role will also involve collaborating with the hospital Care Coordination team to develop discharge plans and providing follow-up consultation for discharged patients.

Key Responsibilities
  • Develop and implement LVAD Program processes, guidelines, policies, and procedures.
  • Collaborate with the hospital Care Coordination team to develop discharge plans.
  • Provide follow-up consultation for discharged patients.
  • Participate in outpatient AHF and LVAD clinic visits in collaboration with medical providers.
  • Provide education and support to nursing staff and patient/caregiver on patient care, maintaining LVAD equipment, and safety measures.
  • Coordinate multi-disciplinary evaluation for patients who have been identified for possible candidacy for LVAD implantation for Destination Therapy/Bridge to Transplant Candidacy.
  • Coordinate referral with partnering medical facility(ies) for patients requiring evaluation for Bridge to Transplant or Transplant.
Requirements
  • Minimum 2 years clinical experience in VAD Coordinator role.
  • Preferred for clinically qualified candidates who hold an associates degree in nursing, for consideration of the LVAD Coordinator role, he or she must commit to completing a Bachelor's Degree in Nursing (BSN) within 24 months of starting the role.
  • Excellent written and verbal communication skills and proficiency in Microsoft Office Suite Products.
