We are seeking a highly skilled LVAD Program Coordinator RN to join our team at Wellstar Health System. As a key member of our cardiovascular team, you will play a critical role in implementing LVAD program policies and procedures, working collaboratively with our Advanced Heart Failure MD and APPs to provide exceptional patient care.
Key Responsibilities- Implement LVAD program policies and procedures, ensuring compliance with state, federal, and organizational guidelines.
- Collaborate with the AHF MD and APPs to provide follow-up consultation for discharged patients and participate in outpatient AHF and LVAD clinic visits.
- Provide education and support to nursing staff, patient/caregiver, and community health care professionals on patient care, LVAD equipment maintenance, and safety measures.
- Coordinate multi-disciplinary evaluation for patients identified for possible candidacy for LVAD implantation for Destination Therapy.
- Coordinate referrals with partnering medical facilities for patients requiring evaluation for Bridge to Transplant or Transplant.
- Document patient information and maintain accurate records.
- Participate in data management and review, identifying adverse trends and reporting on them.
- Act as a consultant within Wellstar Health System and the LVAD community, providing support and education to nursing staff and other health professionals.
- Graduate from an accredited school of nursing.
- Bachelor's Degree in Nursing required.
- Excellent written and verbal communication skills and proficiency in Microsoft Office Suite Products.
- Ability to implement change, collect, analyze, and present data.
