We are seeking a highly skilled Advanced Heart Failure Cardiology Specialist to join our team at Wellstar Health System. As a key member of our cardiovascular care team, you will be responsible for managing and coordinating the healthcare of patients with advanced heart failure in both inpatient and ambulatory settings.
Responsibilities- Manage and coordinate the healthcare of patients with advanced heart failure in both inpatient and ambulatory settings.
- Collaborate with our multidisciplinary clinical team, including cardiologists, surgeons, RN coordinators, and social workers, to provide comprehensive care to patients with heart failure, chronic inotropic support, and durable LVAD therapy.
- Participate in the development and implementation of treatment plans for patients with advanced heart failure, including the use of implantable pulmonary artery pressure monitors, baroreceptor activation therapy, and cardiac contractility modulation.
- Provide education and support to patients and their families on heart failure management and self-care.
- Stay up-to-date with the latest advancements in cardiovascular care and incorporate evidence-based practices into patient care.
- Board Certified/Board Eligible Acute Care Nurse Practitioner/Physician Assistant.
- Previous APP experience in cardiology and AHF is preferred.
