Job Summary

We are seeking a highly skilled and experienced LVAD Program Coordinator RN to join our team at Wellstar Health System. As a key member of our cardiac care team, you will play a critical role in implementing LVAD program policies and procedures, working collaboratively with our Advanced Heart Failure MD and APPs to provide exceptional patient care.

Key Responsibilities
  • Implement LVAD program policies and procedures, ensuring compliance with state, federal, and organizational guidelines.
  • Collaborate with the Advanced Heart Failure MD and APPs to provide follow-up consultation for discharged patients and participate in outpatient AHF and LVAD clinic visits.
  • Provide education and support to nursing staff and patient/caregivers on patient care, LVAD equipment maintenance, and safety measures.
  • Coordinate multi-disciplinary evaluations for patients identified for possible candidacy for LVAD implantation for Destination Therapy.
  • Coordinate referrals with partnering medical facilities for patients requiring evaluation for Bridge to Transplant or Transplant.
  • Document patient information and maintain accurate records.
  • Participate in data management and review, identifying adverse trends and reporting on them.
  • Act as a consultant within Wellstar Health System and the LVAD community, providing education and support to nursing staff and other health professionals.
Requirements
  • Graduate from an accredited school of nursing.
  • Bachelor's Degree in Nursing required.
  • Excellent written and verbal communication skills and proficiency in Microsoft Office Suite products.
  • Ability to implement change, collect, analyze, and present data.
