Position Overview

Under the guidance of the Program Director and in partnership with the Advanced Heart Failure (AHF) physician, the LVAD Program policies and procedures are implemented effectively. This role involves close collaboration with the AHF physician and/or Advanced Practice Providers (APPs), including the APP LVAD Coordinator. The coordinator plays a crucial role in the clinical management of patients before and after LVAD implantation, working alongside healthcare providers. Responsibilities include coordinating the clinical assessment of potential LVAD candidates and ensuring comprehensive discharge plans while confirming patient understanding prior to discharge. Follow-up consultations for discharged patients and participation in outpatient AHF and LVAD clinic visits are also key components of this role.

Additionally, the coordinator is responsible for educating nursing staff and patients/caregivers on patient care, LVAD equipment maintenance, and safety protocols. The role extends to providing education and support to community healthcare professionals who may interact with LVAD patients. Acting as a clinical expert and consultant within the WellStar Health System, the coordinator ensures adherence to state, federal, and organizational regulations, along with other relevant guidelines. Other related duties may be assigned as necessary.

Key Responsibilities

  • Oversee the clinical evaluation of potential candidates for ventricular assist device implantation.
  • Coordinate multi-disciplinary assessments for patients identified as potential candidates for LVAD implantation for Destination Therapy.
  • Facilitate referrals with partnering medical facilities for patients needing evaluations for Bridge to Transplant or Transplant.
  • Participate in the LVAD Patient Selection Committee.
  • Deliver pre-implant education to patients and caregivers; ensure documentation.
  • Provide post-implant education to patients and caregivers; ensure documentation.
  • Develop and utilize patient educational guidelines for the device, reviewing and updating them annually as needed.
  • Conduct community education sessions.
  • Provide post-operative education to patients and caregivers in collaboration with clinical staff to prepare for hospital discharge.
  • Serve as a resource for inpatient and outpatient team members and providers regarding LVAD care.
  • Support patients and caregivers post-discharge by offering clinical and technical assistance.
  • Maintain a coordinator-to-patient ratio of no greater than 15:1 for implanted patients.
  • Document relevant information in patients' medical records.
  • Engage in data management and review, identifying adverse trends and providing conclusions and recommendations based on data analysis.
  • Maintain a patient tracking system and oversee required regulatory and program-specific data.
  • Act as a consultant within the WellStar Health System and the LVAD community.
  • Identify competency and learning needs, collaborating with LVAD leadership and team members to design orientation and ongoing educational activities related to LVAD patient care.
  • Provide support and education to nursing staff and other healthcare professionals regarding the device and troubleshooting alerts and alarms.
  • Under the direction of LVAD leadership and providers, develop educational materials and conduct training sessions for team members and LVAD staff.

Minimum Education Requirements

Graduate from an accredited nursing program is required, along with a Bachelor's Degree in Nursing.

Licensure and Certification Requirements

Registered Nurse (Single State) is required, along with RN - Multi-state Compact and certifications in Basic Life Support and Advanced Cardiac Life Support.

Experience Requirements

A minimum of 2 years of clinical experience in a VAD Coordinator role is preferred.

Skills Requirements

Strong written and verbal communication skills, proficiency in Microsoft Office Suite, and the ability to organize and lead meetings effectively. The candidate must demonstrate the capability to work independently, delegate tasks, and manage multiple responsibilities in a fast-paced environment. Additionally, the ability to implement change, collect, analyze, and present data is essential.
