Position Overview

Under the guidance of the Program Director and in partnership with the Advanced Heart Failure (AHF) physician, the LVAD Program Coordinator is responsible for executing program policies and procedures. This role involves close collaboration with the AHF physician and/or Advanced Practice Providers (APPs), including the APP LVAD Coordinator. The coordinator plays a vital role in the clinical management of patients before and after LVAD implantation, working alongside healthcare providers. Responsibilities include assessing potential LVAD candidates and coordinating discharge plans while ensuring patients are well-informed prior to leaving the facility. Follow-up consultations for discharged patients and participation in outpatient AHF and LVAD clinic visits are also essential aspects of this role.

In addition, the coordinator is tasked with educating nursing staff and patients/caregivers on patient care, LVAD equipment maintenance, and safety protocols. The role extends to providing education and support to community healthcare professionals who may interact with LVAD patients. As a clinical expert, the coordinator serves as a consultant to nursing staff and team members within WellStar Health System, ensuring adherence to state, federal, and organizational regulations.

Key Responsibilities

Essential Functions Include:

  • Oversee the clinical evaluation of potential candidates for ventricular assist device implantation.
  • Coordinate multidisciplinary assessments for patients identified as potential candidates for LVAD implantation.
  • Facilitate referrals with partner medical facilities for patients needing evaluations for Bridge to Transplant or Transplant.
  • Participate in the LVAD Patient Selection Committee.
  • Deliver pre-implant education to patients and caregivers, ensuring thorough documentation.
  • Provide post-implant education to patients and caregivers, maintaining accurate records.
  • Develop and implement patient educational guidelines for device management.
  • Conduct annual reviews and updates of educational materials as necessary.
  • Offer community education regarding LVAD care.
  • Assist in post-operative education for patients and caregivers in collaboration with clinical staff to prepare for discharge.
  • Act as a resource for inpatient and outpatient team members and providers regarding LVAD care.
  • Maintain a patient-to-coordinator ratio of no greater than 15:1.
  • Document relevant information in patients' medical records and participate in data management and review.
  • Identify adverse trends and provide reports with conclusions and recommendations based on data analysis.
  • Maintain oversight of required regulatory and program-specific data.
  • Consult within WellStar Health System and the LVAD community to identify educational needs and develop ongoing training activities.
  • Support nursing staff and other healthcare professionals with troubleshooting and device management education.
  • Collaborate with LVAD leadership to create educational materials and conduct training sessions for team members.

Minimum Qualifications

Graduate from an accredited nursing program is required, along with a Bachelor's Degree in Nursing.

Licensure and Certifications

Registered Nurse (Single State) is required, along with RN - Multi-state Compact and certifications in Basic Life Support and Advanced Cardiac Life Support.

Experience Requirements

A minimum of 2 years of clinical experience in a VAD Coordinator role is preferred.

Skills and Competencies

Strong written and verbal communication skills, proficiency in Microsoft Office Suite, and the ability to organize and lead meetings effectively. The candidate must demonstrate independence, delegation skills, and the ability to multitask in a dynamic environment. Capability to implement changes, collect, analyze, and present data is essential.
