The Care Coordinator plays a pivotal role in the management and organization of care services for young individuals facing serious emotional challenges, specifically those under the age of 21. This position is dedicated to ensuring that essential services are effectively accessed, coordinated, and delivered in a manner that is individualized, family-oriented, and culturally sensitive. The approach is rooted in a Wraparound planning methodology, which results in a tailored and adaptable care plan for both the youth and their family.
Key Responsibilities
- Document detailed progress notes that comply with federal, state, and organizational standards.
- Conduct thorough home-based evaluations and behavioral health assessments, including the Massachusetts Child and Adolescent Needs and Strengths (CANS) tool, in a location chosen by the family.
- Manage a caseload of 11-14 families, ensuring personalized attention and support.
- Facilitate the formation of the Care Planning Team and guide the creation of an Individual Care Plan along with a risk management/safety strategy.
- Oversee the quality and delivery of services to families, acting as a liaison for any new services or resources required.
- Arrange transportation for clients to various appointments and crisis services as necessary.
- Provide or coordinate support and emergency services for youth and their families.
- Collaborate with other professionals involved in the youth's care to ensure comprehensive service coordination.
- Identify community resources and foster natural support systems for youth and their caregivers to help achieve the goals outlined in the individual care plan.
- Minimum of 1 year of relevant experience.
- Educational Requirement: An MCE-approved Master's or Bachelor's Degree in fields such as Family Therapy, Counseling, Human Services, or Community Psychology.
- A valid driver's license and appropriate auto insurance are mandatory.
- Demonstrated awareness and sensitivity to diverse contextual factors including race, culture, gender, sexuality, disability, and socioeconomic status.
