Care Coordinator Job Description

We are seeking a highly skilled and compassionate Care Coordinator to join our team at Gandara Mental Health Center. As a Care Coordinator, you will play a critical role in facilitating care planning and coordination of services for youth with serious emotional disturbance (SED) under the age of 21.

Key Responsibilities:
  • Develop and implement individualized care plans that address the unique needs and goals of each youth and family.
  • Conduct comprehensive home-based assessments and behavioral health assessments to identify strengths and challenges.
  • Maintain a caseload of 11-14 families and provide regular progress updates and support.
  • Collaborate with other professionals, including family partners, to ensure a comprehensive and coordinated approach to care.
  • Identify and connect families with community resources and natural supports to promote sustainability and achievement of care plan goals.
Requirements:
  • Master's or Bachelor's degree in a relevant field, such as family therapy, counseling, or human services.
  • At least 1 year of experience in care coordination or a related field.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Awareness and sensitivity to contextual variables, including race, culture, gender, sexuality, disability, economics, and lived experience.
