Care Coordinator Specialist

At Gándara Mental Health Center, we are seeking a highly skilled and compassionate Care Coordinator to join our team. As a Care Coordinator, you will play a vital role in facilitating care planning and coordination of services for youth with serious emotional disturbance (SED) under the age of 21.
Key Responsibilities:- Submit detailed progress notes that meet federal, state, and agency requirements.
- Conduct comprehensive home-based assessments and behavioral health assessments, including the Massachusetts Child and Adolescent Needs and Strengths (CANS).
- Maintain a caseload of 11-14 families and oversee the assembly of the Care Planning Team.
- Guide the development of an Individual Care Plan and a risk management/safety plan.
- Monitor the provision and quality of services provided to the family and serve as a liaison for new services/resources.
- Provide or arrange for transportation for clients to appointments, crisis/respite services, and other necessary services.
- Collaborate with other necessary individuals the youth and family may have contact with to properly coordinate care.
- Identify community resources and develop natural supports for youth and parent/caregiver(s) to support and sustain achievement of the youth's individual care plan, goals, and objectives.
- Minimum 1 year of experience in a related field.
- Bilingual and Bicultural candidates are encouraged to apply, with Nepalese language preferred.
- Must possess an MCE-approved Master's or Bachelor's Degree in Family Therapy, Counseling and Psychological Services, Human Services, Community Psychology, or a related field.
- A driver's license and proper auto insurance are required.
- Awareness and sensitivity to contextual variables such as race, culture, gender, sexuality, disability, economics, and lived experience.
