Care Coordinator Specialist

We are seeking a highly skilled and compassionate Care Coordinator to join our team at Gandara Mental Health Center. As a Care Coordinator, you will play a critical role in facilitating care planning and coordination of services for youth with serious emotional disturbance (SED) under the age of 21.
Key Responsibilities:- Develop and implement individualized care plans that address the unique needs and goals of each youth and family.
- Conduct comprehensive home-based assessments and behavioral health assessments to identify strengths and challenges.
- Maintain a caseload of 11-14 families and provide regular progress updates and support.
- Collaborate with other professionals, including family partners, to ensure a comprehensive and coordinated approach to care.
- Identify and connect families with community resources and natural supports to promote sustainability and achievement of care plan goals.
- Master's or Bachelor's degree in a related field, such as family therapy, counseling, or human services.
- Minimum 1 year of experience in care coordination or a related field.
- Bilingual and bicultural candidates are encouraged to apply, with a preference for Nepalese language speakers.
- Awareness and sensitivity to contextual variables such as race, culture, gender, sexuality, disability, economics, and lived experience.
