Job Title: Care Coordinator

We are seeking a dedicated and compassionate Care Coordinator to join our team at Behavioral Health Network. As a Care Coordinator, you will play a vital role in supporting individuals recovering from substance abuse and providing guidance and direct counseling to help them achieve their goals.

Key Responsibilities:
  • Provide individual and group counseling to support individuals' growth, independence, and safety.
  • Develop and facilitate treatment plans and diverse psycho-educational groups.
  • Provide crisis intervention and emergency services as needed.
  • Assist individuals with aftercare planning by providing referrals and access to appropriate agencies.
  • Provide transportation for individuals as needed.
  • Complete program-specific documentation, including intakes, treatment plans, comprehensive assessments, aftercare plans, discharge summaries, and counseling notes.
  • Collaborate with Holyoke TSS staff and outside providers to deliver high-quality, sound clinical treatment.
Requirements:
  • High School Diploma or G.E.D. equivalent.
  • At least 1-2 years of experience in human services.
  • At least one year of experience working in a substance treatment environment.
  • Experience providing individual and group counseling.
  • Familiarity with Internet, word processing, and email systems.
  • Working knowledge of DPH Bureau of Substance Abuse regulations.
  • Valid Driver's License and car available for work purposes.
