We are a growing business that values a family-oriented culture and is looking for a dedicated and organized Care Coordinator to join our team in Holyoke.

Job Summary

We are seeking a bilingual Care Coordinator to supervise and manage field staff, build and maintain schedules, and provide excellent customer service to our clients and caregivers.

Key Responsibilities
  • Supervise and manage field staff, ensuring timely and accurate schedule changes and communication.
  • Select caregivers for clients based on care plans and caregiver skills, availability, and attributes.
  • Provide exceptional customer service, compassion, and professionalism in all internal and external interactions.
  • Develop and maintain relationships with caregivers to foster their capacity to provide high-quality care.
  • Participate in the on-call rotation, including nights and weekends, and work productively with Corporate Operations.
  • Track and report call-outs, concerns, and inappropriate behavior in a timely manner.
  • Assist with interviewing and hiring caregivers.
Requirements
  • High school diploma or equivalent.
  • Bilingual (Spanish/English).
  • 2 years of experience in an office environment, with home health care experience preferred.
  • Ability to work in an office Monday-Friday 8AM-4:30PM and participate in the on-call rotation.
  • Strong attention to detail and accuracy in work.
  • Comfortable using Microsoft-based computer applications and internal databases.
  • Establish and maintain good working relationships and communication with stakeholders, colleagues, and field staff.
  • Maintain confidentiality with employee and client information.
  • Professional attitude, demeanor, and dress.
  • Positive attitude, willingness to learn, and desire to be an effective team member.
