Job Overview

Position Summary:

The role involves delivering specialized social work services within the Community Alternatives Program aimed at supporting disabled adults and children. This includes aiding in the identification, coordination, and monitoring of essential social, medical, and financial services tailored to the needs of program participants.

Key Responsibilities:

  • Assessment and Evaluation:
  • Identify patients requiring social intervention and facilitate referrals.
  • Conduct initial screenings and assessments for patients and their families.
  • Evaluate the needs and available resources for patients and families, including the necessity for additional community services.
  • Planning:
  • Develop the social work component of care plans to ensure patient health, safety, and well-being.
  • Review and revise care plans at least annually or as patient needs change.
  • Assist patients and families in comprehending care plans and making informed decisions.
  • Utilize community resources effectively and initiate necessary referrals.
  • Provide counseling and emotional support to enhance the patient/family support network.
  • Implementation and Follow-up:
  • Collaborate with various disciplines within the organization to ensure a holistic approach to patient care.
  • Continuously evaluate and monitor services through thorough documentation and consultations.
  • Engage in relevant interagency meetings.
  • Act as a liaison between patients and service provider agencies.
  • Conduct systematic patient assessments through regular communication and home visits, documenting findings in accordance with policy.
  • Assess and document the effectiveness of care based on observable patient responses, recommending necessary adjustments to achieve desired outcomes.
  • Provide ongoing case management for assigned patients.
  • Documentation:
  • Complete all required documentation in line with agency policies and state regulations.
  • Document significant changes in patient conditions during interactions with patients, families, and other service providers.
  • Prepare discharge summaries upon completion of CAP services, summarizing provided services and patient status at discharge.
  • Maintain comprehensive medical records for each patient, ensuring current documentation of status, service changes, and referrals.
  • Ensure case management documentation is accurately maintained within the designated system, review aide documentation, and oversee billing compliance according to guidelines.
  • Professional Development:
  • Fulfill continuing education requirements as established by agency protocols.
  • Complete state-mandated training and updates as required.
  • Collaborate with the Community Alternatives Program Director to set and achieve professional development goals, revising them as necessary to reflect the evolving nature of the unit.
  • Participate in mandatory organizational training as per policy.

Qualifications:

Skills and Knowledge:

  • Proficient understanding of fundamental social work principles, techniques, and their application to specific casework and community challenges.
  • Familiarity with governmental and private organizations and resources available in the community.
  • Strong organizational, communication, listening, and assessment capabilities.
  • Willingness to travel as required to various office locations.

Education and Training:

  • Bachelor's degree in social work from an accredited institution with one year of relevant community experience, preferably in case management within the health or medical sector, and completion of an NC Medicaid certified training program within 90 days of employment; OR
  • Bachelor's degree in a human services field with two or more years of relevant community experience in health or medical case management, and completion of an NC Medicaid certified training program within 90 days; OR
  • Bachelor's degree in a non-human services field with two or more years of relevant community experience in health or medical case management, and completion of an NC Medicaid certified training program within 90 days; OR
  • A nurse with a current North Carolina license and two or four-year degrees, along with one year of case management experience in home care, long-term care, or related fields, and completion of an NC Medicaid certified training program within 90 days; OR
  • An individual with a bachelor's degree or nursing license as described above, without the requisite years of experience, may be designated as an apprentice or trainee under direct supervision.

Physical and Environmental Requirements:

This position may require prolonged periods of sitting while using a computer, along with frequent stretching, talking, listening, bending, and reaching. Occasionally, it may involve moving supplies or equipment to patient residences and navigating stairs. There may be exposure to various environmental factors that could pose safety risks.
