Position Summary:
The role involves delivering specialized social work services within the Community Alternatives Program aimed at supporting Disabled Adults and Children. This includes aiding in the identification, coordination, monitoring, and assessment of eligibility for various social, medical, financial, and additional services tailored to meet the needs of patients enrolled in the Community Alternative Program.
Key Responsibilities:
- Assessment and Evaluation: Identify patients requiring social intervention and referrals, conduct initial screenings and assessments for patients and their families, and evaluate their needs alongside available community resources.
- Planning: Formulate the social work segment of the care plan to ensure the health, safety, and overall well-being of patients. Regularly review and update care plans as necessary, assisting families in understanding their options.
- Implementation and Follow-up: Collaborate with various disciplines within the organization to provide a holistic approach to patient care. Maintain ongoing evaluations and documentation of services provided, participating in interagency meetings as needed.
- Documentation: Ensure all records are completed in accordance with agency policies and relevant guidelines, documenting any significant changes in patient conditions during interactions.
- Professional Development: Engage in continuous education and training as mandated by agency policies, working with the Community Alternatives Program Director to set and review professional goals annually.
Qualifications:
- Skills and Knowledge: Proficient understanding of social work principles and their application in various contexts, familiarity with community resources, and strong organizational and communication skills.
- Education/Training: A Bachelor's degree in social work or a related field, with relevant community experience in healthcare settings. Completion of required training programs is essential.
