We are in search of a dedicated Nutrition Specialist to become a crucial member of our interdisciplinary care team (IDT), committed to making a positive impact on our members' lives every day. The PACE model enables us to focus on addressing underlying causes and environmental influences rather than merely treating symptoms.

Compensation for the Role:

This is a mid-level position with opportunities for career growth. We are flexible with part-time or full-time arrangements as our program expands. The salary range is $52,000 - $72,800, with various opportunities for advancement.

Key Responsibilities:

The primary role of the Nutrition Specialist is to advocate for our members and actively participate in the IDT. Below are the essential duties:

  • Assess the nutritional status of participants through various lab values and physical evaluations, obtaining necessary orders from primary care providers.
  • Evaluate participants' food preferences, preparation methods, and cultural factors that influence dietary choices.
  • Contribute to the formulation and updates of each participant's care plan as part of the IDT.
  • Create a comprehensive nutrition plan tailored to the participant's medical and physical needs, integrating it into the overall care strategy.
  • Educate families and caregivers on the preparation and importance of therapeutic diets as part of the treatment plan.
  • Continuously review and adjust nutritional plans to ensure they remain effective and appropriate, coordinating with primary care providers as needed.
  • Communicate treatment responses to the IDT and primary care providers.
  • Guide food selection and preparation to align with the overall nutrition treatment plan.
  • Oversee meal services, including home-delivered meals and sanitation, while supervising support staff in their nutritional service duties.
  • Compile and analyze records, reports, and statistical data for program evaluation and planning.
  • Maintain accurate and timely documentation of all services rendered.
  • May participate in team meetings to discuss the current nutrition treatment plan and provide feedback for updates or revisions.

Qualifications:

Education: Bachelor’s Degree in Nutrition or a related field; Master’s Degree preferred.

Licensure/Certification: Required licensure or certification for Dietitians or Nutritionists.

Experience:

  • A minimum of 2 years of experience in nutritional counseling and education.
  • At least one year of professional experience working with elderly or frail populations.
  • Ability to collaborate effectively within an interdisciplinary team.
  • Experience working with diverse populations across various cultural, economic, and educational backgrounds.
  • Strong interpersonal skills to build positive relationships with staff, external contacts, participants, and their families.
