Position Overview

The Healthcare Program Coordinator plays a vital role within a multidisciplinary team dedicated to enhancing the health and wellness of the American Indian/Alaska Native community. This position is responsible for orchestrating health and nutrition education initiatives in a community-centered healthcare environment.

Key Responsibilities

  • Conduct comprehensive nutrition assessments and develop tailored care plans for patients, ensuring the provision of Medical Nutrition Therapy.
  • Deliver nutrition education aimed at the prevention and management of diabetes.
  • Facilitate individual and group health education sessions, creating and implementing follow-up strategies to support behavior modification.
  • Ensure effective communication and information transfer among team members and other healthcare professionals to streamline patient care.
  • Oversee the management of referrals for nutrition and diabetes services.
  • Maintain meticulous documentation of all patient interactions, utilizing appropriate coding for data management.
  • Provide case management for diabetic patients, assisting healthcare providers with insulin management.
  • Coordinate community outreach programs and health education materials.
  • Manage grants and contracts related to program operations, including budget oversight and deliverable tracking.
  • Support administrative functions required by funding sources, including documentation and reporting.
  • Engage with community partners to enhance program outreach and effectiveness.
  • Keep the Healthcare Clinical Operations Director informed about program activities and potential challenges.
  • Participate in departmental meetings and contribute to team discussions.
  • Perform additional duties as necessary to fulfill the role's objectives.

Qualifications

  • Bachelor's degree in dietetics or a related field from an accredited institution.
  • A minimum of two years of experience in health education and community clinic settings.
  • Proficiency in motivational interviewing and behavior change counseling.
  • Current California CDR Registration and willingness to obtain CDE designation if not already held.

Work Environment

The role is primarily office-based, with occasional community engagement activities. Employees may experience varying noise levels and will be required to perform physical tasks such as standing, walking, and lifting up to 25 pounds.
