Job Summary:

The Care Coordinator for Harm Reduction coordinates client intakes and non-clinical services at our shelter- and street-based services. This role requires a strong understanding of harm reduction principles and a commitment to providing patient-centered care.

Key Responsibilities:
  • Screen clients for social determinants of health using validated screening instruments.
  • Develop and evaluate shared plans of care for patients.
  • Liaise with organizations and care providers to link clients to available services.
  • Link patients with social needs to community resources.
  • Assist with and follow-up on health maintenance items and chronic disease management.
