Job Summary

We are seeking a skilled and compassionate Triage Nurse to join our team at Fair Haven Community Health Care. As a key member of our nursing staff, you will play a critical role in ensuring the high-quality care of our patients by assessing their health needs, providing triage services, and coordinating patient flow.

Responsibilities
  • You will be responsible for managing nurse-directed patient telephone calls and walk-in patients, evaluating patient health needs based on signs and symptoms, and obtaining input from providers as needed to determine the best course of action.
  • You will accurately document patient information, including symptoms, assessment, advice provided, and patient response. You will also coordinate same-day and walk-in appointments on provider schedules and utilize standing orders, triage protocols, and algorithms appropriately.
  • You will monitor patient flow in the clinic, ensuring that patients receive all recommended services in a timely manner. Additionally, you will perform other nursing functions as assigned, assist providers with special procedures as needed, and administer certain medications and injections as ordered by providers.
Requirements

To be successful in this role, you must have a current unrestricted RN license in CT, a BSN degree preferred, and one year of triage experience. Fluency in Spanish is also desired. You should possess knowledge of disease processes, normal growth and development, and community health theory. You must operate with cultural sensitivity and respect for diverse racial, ethnic, economic, and social backgrounds. You should also have general computer skills and experience with EHR systems, specifically Epic. You will need to demonstrate excellent communication skills, clinical competence, and the ability to handle stressful emergency situations calmly and effectively.

Benefits

We offer a competitive salary range of $62,000 - $80,000 per year, depending on experience. In addition, you will enjoy a comprehensive benefits package, including medical, dental, and vision coverage, retirement savings plan, and paid time off.
