Responsibilities:
* Evaluate patient needs and develop individualized care plans
* Coordinate care with healthcare teams to ensure seamless transitions
* Deliver high-quality nursing care, considering cultural, psychological, functional, cognitive, and social determinants
* Maintain accurate records and documentation in EMRs

Requirements:
* Current State of Oklahoma Registered Nurse License
* Valid driver's license and reliable automobile for transportation as needed
* Minimum 1 year of nursing experience, with hospice experience preferred
* Excellent communication, interpersonal, and organizational skills

Salary: $63,000 - $80,000 per year, depending on experience
