About This Role

We are seeking a highly skilled Oncology Multispecialty Care Coordinator to join our team. As an integral member of our Disease Team, you will be responsible for navigating patients and their families across care settings, ensuring that they have access to appropriate information and understand their medical plan.

In this role, you will serve as a consistent primary contact for new patients and their families, partnering with our Intake Assistant to expedite entry of new patients into the system. You will also collaborate with Disease Team members to ensure coordination of care across the continuum for designated patient populations.

The Oncology Multispecialty Care Coordinator will synchronize patient/family care across care settings, beginning with the initial referral and consultative visit and ending with handoff at a designated point determined by the specific disease team. You will interface with point of care staff in accordance with the plan for patient care, educate patients and families based on determination of learning needs, and communicate plan detail to patient/family, physicians, mid-level providers, and others involved in the patient's care.

Qualifications:

To be successful in this role, you will need a BSN degree and 3 years of oncology or surgical nursing experience. A Connecticut RN license is required, and Oncology Nursing Society certification (OCN, AOCN) is desirable.

Compensation:

The estimated salary for this role is $83,500 - $114,000 per year, depending on experience.
