Job Summary

We are seeking a highly skilled Patient Care Coordinator to join our team at Yale-New Haven Health. As a key member of our nursing staff, you will play a vital role in providing quality care to our residents.

About the Role
  • Provide personalized care to residents based on their individual needs and preferences.
  • Collaborate with Charge Nurses to ensure seamless transitions and efficient delivery of care.
  • Assist with daily living activities, such as bathing, dressing, and grooming, while promoting independence and dignity.
  • Monitor resident's vital signs, including temperature, pulse, and blood pressure, and report any abnormalities to the Charge Nurse.
  • Administer medications and treatments as prescribed by physicians and report any adverse reactions.
  • Document resident's progress, including medical history, treatment plans, and outcomes.
  • Communicate effectively with residents, families, and healthcare teams to ensure comprehensive care.
Qualifications
  • Certified Nursing Assistant (CNA) certification or equivalent experience required.
  • High school diploma or equivalent education required.
  • Ability to work effectively in a fast-paced environment and prioritize tasks efficiently.
  • Excellent communication and interpersonal skills necessary for effective collaboration with healthcare teams.
  • Demonstrated ability to maintain confidentiality and handle sensitive information with discretion.
Salary and Benefits

The estimated salary for this position is $45,000 - $55,000 per year, depending on experience and qualifications. Additionally, we offer a comprehensive benefits package, including health insurance, paid time off, and retirement savings plan.
