Job Summary

We are seeking an experienced Registered Nurse to join our team as a Nursing Care Coordinator. In this role, you will have the opportunity to work collaboratively with patients, families, and healthcare teams to deliver personalized care that meets the unique needs of each individual.

Duties and Responsibilities
  • Provide high-quality patient care that is guided by evidence-based practices and standards of professional performance
  • Collaborate with interdisciplinary teams to develop and implement comprehensive care plans that address the physical, emotional, and social needs of patients
  • Evaluate patient progress and outcomes, making adjustments to care plans as needed to ensure optimal results
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless transitions of care and promote continuity of care
Requirements
  • Graduation from an accredited school of nursing with a BSN degree preferred
  • Licensure as a registered nurse in the State of Connecticut
  • Current BCLS certification or ability to obtain within one month of hire
