Medical Director for Home Health Services

We are seeking a highly skilled Nurse Practitioner to lead our home health services team at Abode Care Partners. As a Medical Director, you will be responsible for providing personalized medical care to senior patients and those with complex needs in various home settings.

About the Role:
  • Lead a team of healthcare professionals in delivering evidence-based medical care to patients in their homes.
  • Develop and implement comprehensive care plans tailored to individual patient needs.
  • Collaborate with caregivers and other healthcare providers to ensure seamless care coordination.
Responsibilities:
  • Perform routine medical assessments and develop treatment plans for patients with chronic illnesses and acute conditions.
  • Treat minor injuries, accidents, and episodic illnesses in a home setting.
  • Order and interpret laboratory and diagnostic testing as needed to inform patient care decisions.
  • Collaborate with interdisciplinary teams to provide comprehensive care and support.
Requirements:
  • Active state Nurse Practitioners license and current certification.
  • Minimum 5 years of nursing experience, including at least 2 years as a Nurse Practitioner.
  • Strong interpersonal communication skills and ability to work effectively in a team environment.
  • Proficiency in electronic medical record systems and data management programs.
  • Licensed driver with valid automobile insurance.
