We are seeking a compassionate Primary Care Provider to join our team. As a member of our interdisciplinary team, you will provide personalized medical care to senior patients and those with complex needs in their homes. Your responsibilities will include performing evidence-based assessments, treating chronic and urgent episodic illnesses, minor injuries, and other health issues. You will also collaborate with healthcare professionals to maintain comprehensive care plans and ensure patient medical records are kept up-to-date.

To succeed in this role, you must have an active state Nurse Practitioner license, current National Board Certification, and active DEA licensure. A Master's degree in Nursing (MSN) and at least five years of nursing experience, with two years as a nurse practitioner, are also required. Strong interpersonal communication skills, excellent time management skills, and proficiency in medical instruments and equipment are essential.

The compensation package for this position includes competitive salary, flexible schedule, weekdays, and daytime shifts only, mileage reimbursement, employee referral program, professional licensure reimbursement, tuition reimbursement, paid time off, 401(k) retirement savings plan, medical, dental, and vision benefits, life insurance, Flexible Spending Account (FSA), and employee discounts.

Salary Range: USD $105,000.00 - $120,000.00 / Year
