At Abode Care Partners, we are seeking a skilled Nurse Practitioner to join our team of dedicated healthcare professionals. This is an excellent opportunity for a licensed nurse practitioner to provide personalized medical care to senior patients and those with complex needs in various home settings.

Responsibilities- Perform evidence-based assessments and develop comprehensive care plans that address the unique needs of each patient
- Treat chronic and urgent episodic illnesses, minor injuries, accidents, and other injuries or illnesses
- Collaborate with caregivers and other healthcare professionals to maintain interdisciplinary care and ensure seamless transitions between care settings
- Order and interpret laboratory and diagnostic testing as needed
- Active state Nurse Practitioners license
- Current Nurse Practitioner National Board Certification
- Active state DEA licensure
- Master's degree in Nursing (MSN)
- Five or more years of nursing experience; at least two years of nurse practitioner experience preferred

What We Offer- Competitive salary: $100,000 - $120,000 per year
