Job Description

Faith Regional Health Services seeks a dedicated and compassionate Registered Nurse (RN) to join our Home Health team.

Job Summary

We are seeking an experienced RN to provide high-quality patient care in the comfort of their own homes. As a Home Health Nurse, you will work closely with patients, families, and healthcare teams to develop and implement individualized care plans that promote optimal health and well-being.

Key Responsibilities
  • Conduct comprehensive assessments to identify patient needs and develop personalized care plans.
  • Provide direct patient care, including wound care, medication management, and education on self-care techniques.
  • Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to ensure seamless care coordination.
  • Maintain accurate and timely documentation of patient progress and outcomes.
Requirements

To be successful in this role, you must possess:

  • A valid Registered Nurse (RN) license in the state of Nebraska.
  • Basic Life Support (BLS) certification within one month of hire.
  • Previous clinical experience preferred.
  • Excellent communication and interpersonal skills.

Faith Regional Health Services offers a competitive salary range of $65,000 - $85,000 per year, depending on experience, as well as benefits that include:
