Home Health Nurse Specialist

We are seeking a skilled Home Health Nurse Specialist to provide personalized medical care to seniors and patients with complex needs in various home settings. The successful candidate will evaluate patient medical needs, treat chronic illnesses, collaborate with healthcare professionals, and maintain updated medical records.
About the RoleThis is an exciting opportunity to work as part of our interdisciplinary team, providing high-quality patient-centered care. As a Home Health Nurse Specialist, you will be responsible for:
- Evaluating patient medical needs and developing comprehensive plans of care.
- Treating chronic illnesses, injuries, and other health conditions.
- Collaborating with caregivers and other healthcare professionals to ensure seamless care delivery.
- Maintaining accurate and up-to-date medical records.

What We OfferWe offer a competitive compensation package, including:
- A salary range of $100,000 - $120,000 per year, based on performance and experience.
- An excellent benefits package, including paid time off, 401(k) retirement savings plan, medical, dental, and vision coverage, and life insurance.

To be successful in this role, you will need:
- A Master's degree in Nursing (MSN).
- An active state Nurse Practitioners license.
- Current Nurse Practitioner National Board Certification.
- Active state DEA licensure.
- Five or more years of nursing experience; at least two years of nurse practitioner experience preferred.
