About Our House Calls Nurse Practitioner Role

At Abode Care Partners, we are seeking a compassionate and skilled Nurse Practitioner (FNP/AGNP) to join our team of dedicated healthcare professionals. This exciting opportunity allows you to provide personalized medical care to seniors and patients with complex needs in the comfort of their own homes.


Key Responsibilities:
  • Deliver evidence-based medical care by assessing, planning, directing, and evaluating patient medical needs.
  • Examine and treat chronic and urgent episodic illnesses, minor injuries, accidents, and other conditions.
  • Order and interpret laboratory and diagnostic testing as needed, ensuring timely and accurate results.
  • Collaborate with caregivers and other healthcare professionals to provide interdisciplinary care and maintain comprehensive plans of care.
  • Refer patients to other healthcare practitioners when necessary, ensuring seamless continuity of care.
  • Maintain accurate and up-to-date patient medical records, adhering to HIPAA and OSHA regulations.

Requirements:
  • A valid state Nurse Practitioners license.
  • Current Nurse Practitioner National Board Certification.
  • An active state DEA licensure.
  • A Master's degree in Nursing (MSN).
  • No less than 5 years of nursing experience, with at least 2 years as a nurse practitioner.
  • Excellent interpersonal communication skills, both verbal and written.
  • Strong time management skills, with accountability for daily workflow.
  • Proficiency in medical instruments and equipment.
  • Demonstrated knowledge of HIPAA and OSHA regulations.
  • Ability to engage in moderate physical activity.
