About the Role

We are seeking a highly skilled Mobile Medical Specialist to join our team at Abode Care Partners. As a key member of our healthcare team, you will provide personalized medical care to senior patients and those with complex needs in various home settings.

Responsibilities
  • Perform evidence-based assessments to identify patient needs and develop comprehensive care plans.
  • Treat chronic and urgent episodic illnesses, minor injuries, and other medical conditions.
  • Collaborate with healthcare professionals to maintain a high level of patient care and satisfaction.
  • Order and interpret laboratory and diagnostic testing as needed.
  • Provide referrals to other healthcare practitioners when necessary.
Requirements
  • Active state Nurse Practitioners license
  • Current Nurse Practitioner National Board Certification
  • Active state DEA licensure
  • Master's degree in Nursing (MSN)
  • Five or more years of nursing experience; at least two years of nurse practitioner experience preferred
  • Strong interpersonal communication skills – verbal and written
  • Excellent time management skills with accountability for practitioner's daily workflow
